- Recently, detection of an incidental hepatic tumor has rapidly increased with the introduction
of multi-modality imaging techniques. And then, it is very important to determine whether
these tumors are malignant or benign lesions. In some cases, differentiation from other hepatic
tumors such as focal nodular hyperplasia or hepatic adenoma and hepatocellular carcinoma
can be extremely difficult, both clinical and radiologic finding. Therefore, it is suggested that
combination of multi-modality imaging study than using only a single imaging test. Despite
advanced imaging studies such as computed tomography (CT), magnetic resonance imaging
(MRI), contrast enhanced ultrasonography, and positron emission tomography (PET) showed
a high sensitivity and specificity, role of liver biopsy is still remained to confirm the diagnosis.
The use of liver biopsy in the diagnosis of hypervascular hepatic tumors is controversial.
However if the tumors was shown rapid growth tendency and heterogeneous radiological
appearance, liver biopsy or surgical resection should be considered to provide final diagnosis
and prompt curative treatment. We report a case of a hepatocelluar carcinoma showing
similar radiologic characteristics to focal nodular hyperplasia in patients without high risk
factors of hepatocelluar carcinoma.

- Advanced hepatocelluar carcinoma (HCC) with portal vein thrombosis has a poor prognosis due to inadequate
tumor response. 1 year survival rate and median surviral of stage IVa is less than 20%, 3 months respectively.
Transarterial chemoembolization (TACE) was known to prolong the overall survival rate in patients with
moderately advanced HCC. But, only small number of patients with stage IVa can receive the TACE. Other
treatment modalities to manage for advanced HCC are systemic chemotherapy, hepatic arterial infusion
chemotherapy or radiotherapy. Especially concurrent chemo-radiotherapy would be a better treatment option. We
report a case of good response after repeated hepatic arterial infusion chemotherapy and radiotherapy for patient
in advanced HCC with portal vein thrombosis.

- A 54 year-old male patient was transferred from pulmonologic division to evaluate hepatic mass incidentally found on chest CT scan. Thirteen years ago, he had been diagnosed as a hepatitis B virus carrier. HBs Ag was positive, and AFP was 118.09 ng/ml. On admission, Chest PA showed multiple variable sized nodular opacities in both lungs. Ill defined inhomogeneous enhanced mass was found at liver dome with multiple metastatic lesions on chest CT scan. Hepatic angiogram showed multiple, abnormal tumor staining on liver dome and nearly entire hepatic segments. After the 2nd TACE using mytomycin C, lipiodol and gelform, previously noted hematogeneous metastatic nodules in both lungs disappeared and volume of liver mass was decreased dramatically and AFP was normalized.
